At your first visit, you can expect a warm welcome to our dental family. The first step is completing a medical and dental history, taking necessary records and completing an assessment of your present dental conditions and treatment options. We will discuss with you the type of treatment plan that would best meet your needs.

*Prior to your first visit, please feel free to access our New Patient forms which include a Medical History Form, Consent for Collection of Personal Information Form, EDI Permission form and Authorization for Release of Records form. Each form is required to be completed and submitted prior to/ at your first appointment.
